Cataract Surgery

A cataract is a cloudy area in the lens in the front of the eye, which is normally clear. There is no pain associated with the condition but there are other symptoms, including:

  • Blurred/hazy vision
  • Spots in front of the eye(s)
  • Sensitivity to glare
  • A feeling of "film" over the eye(s)
Risk factors for developing cataracts include being over 55 years old, eye injury or disease, a family history of cataracts, smoking or use of certain medications.

For people who are significantly affected by cataracts, lens replacement surgery may be recommended. During cataract replacement, the most common surgical procedure in the country, the lens is removed and replaced with an artificial one called an intraocular lens or IOL.

What is LASIK?

LASIK, short for Laser Assisted In Situ Keratomileusis, is a 15-minute laser vision correction procedure, which reduces dependence on corrective lenses or eliminates the need for corrective lenses altogether, depending on patient-specific variables. In the procedure, the LASIK surgeon lifts a tiny flap of tissue on the surface of the eye in order to reshape the underlying cornea through use of a computer-guided excimer laser. The surgeon then repositions the flap of tissue over the reshaped cornea. LASIK is a surgical procedure that is capable of correcting a wide range of nearsightedness (myopia), farsightedness (hyperopia) and astigmatism.

Who can benefit from vision correction surgery?

Does LASIK Hurt?

Orange County Laser Eye Surgeon, Dr. Mark Schneider's response to the above question is:

There is no pain during any of the laser vision correction procedures since anesthetic drops numb the eyes. A relaxation pill is also given prior to the procedure to help relieve anxiety. The only discomfort that may be experienced is a slight pressure sensation lasting about 20 seconds, while the Intralase laser creates the corneal flap. After LASIK you might experience slight irritation, a burning sensation, or increased tearing for 2 to 4 hours. We encourage our patients to rest or sleep during this period of time. If needed, an over the counter pain reliever should be all that is needed to relieve this discomfort. PRK patients may experience more post-operative discomfort for 3 to 5 days following the procedure.

What is a cataract?

A cataract is a gradual clouding of the eye's natural lens. Usually most adults over the age of 60 have symptoms of cataracts. A Cataract can be the reason sharp images become blurry or vision at night becomes more difficult.

What are the symptoms of a cataract?

Because the cloudy lens allows less light to reach the retina, distance and/or reading vision is often fuzzy and indistinct through the affected eye.

Other key symptoms of cataracts:

  • Difficulty reading small print such as the newspaper
  • Difficulty driving, especially at night
  • Annoying glare from sunlight and or car headlights
  • Cloudy or hazy vision
  • Diminished color perception


How are cataracts diagnosed?

  • Medical History - Current medications, any allergies, prior eye disease, and previous surgeries.

  • Eye Exam - Refraction to test prescription, examining the back structures of the eye, including retina and optic nerve, eye pressure is also checked to check for glaucoma.

CustomVue Monovision LASIK

Monovision is used to treat presbyopia which is when the eye loses it's ability to focus from one distance to another. Presbyopia generally occurs in people at around the age of 50. Traditionally monovision was provided through the use of specialized contacts or bifocals.

However, with the advent of LASIK and new technologies, there is now CustomVue Monovision LASIK, which is used to treat one eye to see far away objects and the other eye for close-up vision.

The CustomVue Monovision LASIK provides eye surgeons with a larger array of permanent vision correction options for nearsighted adults who also have trouble focusing on objects close-up. Monovision LASIK also helps reduce the need for adults over 40 to use reading glasses.

The CustomVue device is designed to correct all nearsightedness in the patient's dominant eye and only part of the nearsightedness in the non-dominant eye. By doing that, this allows the patient to use the fully corrected eye for distance vision and the under-corrected eye for seeing close up. The brain adjusts to the difference in perception between the two eyes after some time.

People considering CustomVue monovision LASIK should first wear monovision contact lenses for at least a week to determine if they can tolerate having one eye under-corrected. Following monovision surgery, the two eyes may not work together as well as they did before in some patients, especially in dim light or when performing tasks requiring very sharp vision or fine depth perception. Patients may need to wear glasses or contact lenses for some activities such as night driving or reading small type.

The Original Laser Eye Surgery: PRK (Photorefractive Keratectomy)

Before all the commotion and popularity of LASIK there was another procedure known as PRK or Photorefractive Keratectomy. Although it is an older and outdated procedure, it is still favored by many surgeons to treat specific conditions that traditional LASIK is not suited for like like patients with thin corneas or larger pupils.

Although the FDA approved the first PRK laser in the United States in 1995, the PRK laser was developed in the 80's and has been in use in other countries.

PRK can correct vision for both nearsighted and farsighted people. Nearsightedness is corrected by flattening the steepness in the cornea whereas farsightedness requires the steeper cornea. PRK can also address astigmatism simply by smoothing our the oblong shape of the cornea into a normal one.

LASIK Can Be Part of Emergency Preparedness

According to Major Steven Stetson, MD, an ophthalmologist formerly in the Air Force, LASIK can be an important component of emergency preparedness for people who are dependent on glasses or contact lenses. "LASIK is useful in two ways," Stetson explained. "First is the emergency component: the flooding of New Orleans occurred in the middle of the night and many people did not have time to fumble for their glasses or contact lenses; those who were less dependent on them could move more quickly to safety."
