Why Are There Different Kinds of LASIK?

When conventional LASIK was first developed in the 1990's, it was capable of delivering good enough vision for most people to pass a driver's exam without corrective lenses (20/40). Many of these people were fortunate enough to get an exceptional result of 20/20, but the baseline expectation was 20/40. Technological advances in wavefront guided LASIK and femtosecond laser flap creation have raised the bar so that 20/20 is now the baseline expectation and 20/16 or better is the goal.

New lasik research reveals unexpected finding: Key to better-than-20/20 vision is in the flap

New scientific data being presented at this year's ASCRS meeting reveals the key to a better-than-20/20 outcome in LASIK surgery may depend on whether your doctor uses a blade or a laser to create the corneal flap in the first step of the procedure.

"It turns out that the flap that we make in LASIK is not an innocent bystander," says Roger F. Steinert, M.D., 2005 ASCRS president, professor of ophthalmology, professor of biomedical engineering, director of cornea, refractive and cataract surgery, and vice chair of clinical ophthalmology at University of California, Irvine.

Advanced Surface Ablation

Advanced Surface Ablation is a relatively new laser vision procedure that has additional safety features over LASIK making it an ideal alternative for some patients. Instead of cutting a flap on the front of the cornea, the surface skin cells (epithelium) are loosened with a diluted alcohol solution and removed. The corneal surface is then treated with the laser and covered with a bandage contact lens.

The margin of safety with ASA may be greater than that with LASIK as the need for a microkeratome is eliminated, so there's no risk of flap complications. You do not need to worry about rubbing your eye or bumping it after surgery and having the LASIK flap slip or wrinkle, because there is no flap. The nerve endings that control tear secretion are not cut, so there is less of a problem with dry eye after surgery.

ASA removes tissue within the cornea to treat low to high levels of nearsightedness, farsightedness and astigmatism.

The Epi-LASIK Procedure

The Epi-LASIK procedure begins by cutting the cornea's outer layer, the epithelium, using a plastic blade, called an epithelial separator, to separate the sheet from the eye. After the epithelial flap is created, it is then lifted and gently folded out of the way. An excimer laser is then used to carefully reshape the cornea.

Finally, the epithelial flap is placed back on the eye and a special contact lens is placed over the eye to keep the flap in place while it re-epithelializes. Surgeons use a high-Dk contact lens to protect the surface of the eye and allow the epithelial cells to grow back properly.

What is an excimer laser and how does it work?

The excimer laser produces a high-energy, cold, ultraviolet light beam in pulses and delivers the pulses to a surface of the eye's cornea. These pulses break the bond between molecules and tissue cells so that a controlled amount of tissue can literally be vaporized away to reshape the cornea -- one microscopic layer at a time. The excimer laser is computer controlled and programmed to custom treat each individual eye with a high degree of precision. For most cases, the thickness of the layer removed by the laser is less than the thickness of a human hair.

So How Long Does LASIK Take?

What is Wavefront treatment?

So What Exactly is Refractive Surgery?

How Epi-LASIK Works

How Does the IntraLase Method Work?

The IntraLase Method uses tiny, rapid pulses of laser light to create your corneal flap instead of using a metal blade during the first step of LASIK. Each pulse of light passes through the top layers of your cornea and forms a microscopic bubble at a specific depth and position within your eye that is determined by the doctor.

The IntraLase laser moves back and forth across your eye, creating a uniform layer of bubbles just beneath your corneal surface.

Just prior to applying laser vision correction, the doctor creates your corneal flap by gently separating the tissue where these bubbles have formed. The corneal flap is then folded back so the doctor can perform the second step of your LASIK treatment.

A New Technique for Pre-PRK Epithelium Removal

The laser-scrape method may have several advantages over traditional techniques used to remove epithelium before PRK.

Most surgeons would agree that epithelium removal in photorefractive keratectomy is one of the procedure's most critical aspects. Expedient, uniform removal helps minimize stromal drying, which can increase the ablation rate and lead to overcorrection.

The scrape technique, using a PRK spatula or a blade, is widely used. Some surgeons first loosen the epithelium with alcohol. It is common to remove peripheral epithelium first, then central epithelium, to prevent excessive central drying. Unfortunately, this approach may cause excessive peripheral drying, which could lead to excessive peripheral tissue ablation and a steep central island.

Bye-bye, bifocals? A new three-minute procedure may help baby boomers read without glasses

It happens to everyone sooner or later: You can't read a book unless you hold it at arm's length. You have to ask a friend to help you peruse a menu. Oliver Besnoin noticed his vision was headed south the day the stock market data he obsessively watches scroll across his computer screen was simply too small to read. Even so, he balked at wearing bifocals. "I was not ready for that. That's not me," says the 47-year-old former fasion model, who races motorcycles in his spare time.

CK is not cheap-it costs up to $2,500 and is not covered by insurance. It only works for those who didn't need glasses before they turned 40 and has not been approved for nearsightedness (Myopia), one of the most common vision problems. Nevertheless, experts predict the market for the new and relatively painless method will be huge. "It's another option for patients who want to avoid reading glasses," says Harvard Medical School clinical instructor Kernest Kornmehl, author of an article on CK that is due to appear in an upcoming issue of the New England Journal of Medicine. Unlike LASIK, which uses a laser to slice and reshape the cornea, CK doesn't involve cutting (see box). Instead, a tiny probe applies radio waves in a circular pattern just below the surface of the eye. The radio waves act like a belt reshaping the cornea into a perfect circle, improving its contour and allowing light to focus properly on the back of the eye.
